From the founder of Jay's Heart:
In July of 2023 my dad had another heart attack and his heart was failing. The doctors were offering a ‘balloon pump’ and ‘dialysis’ and my mom was expected to make decisions on these interventions. The information was complicated and rushed. She couldn’t formulate questions to ask.
She needed translation and navigation above and beyond what the doctor had time to give. If my mom couldn’t navigate this process, many other adult decision makers probably also struggle with these same issues.
Our Team and Purpose
Dr. Diane Wintz
Diane Wintz, MD, FACS, founded Jay’s Heart in 2023 as a tribute to her father’s journey through heart disease and palliative care, with a mission to prioritize independence and quality of life during medical transitions. She is board-certified in general and critical care surgery, with an active hospital-based practice that emphasizes health in aging. Her nationally recognized work in geriatric trauma has earned podium presentations, publications, and leadership roles. Her passion remains guiding patients and families through some of life’s most difficult decisions with clarity and compassion.
Kelly Wright
Kelly Wright, MSN, MBA, RN, CHPN, OCN, is a registered nurse and coordinator for medical navigation services at Jay’s Heart. She is board-certified in oncology, hospice, and palliative care and leads efforts to provide whole-person care by educating patients, identifying priorities, and ensuring resources are in place.
Wright also serves as a hospital-based nurse manager, where she champions programs that honor older adults’ wishes and improve care transitions. Her leadership has driven measurable improvements, and her team engages with thousands of patients annually to establish goals of care and assess surgical risk. A respected educator and speaker, Wright has presented at national and international conferences on palliative care and compassionate communication.
The End of Life deserves as much beauty, care and respect as the beginning.
